Ralph Shafer was admitted into a nursing home in September of 2007 after suffering two strokes and exhibiting signs of dementia, hypertension and diabetes. While in the nursing home he suffered from frequent falls. Mr. Shafer broke his hip and had to undergo surgery because of one of the falls he took. The facility also failed to prevent the deterioration of a diabetic ulcer on his ankle.

Mr. Shafer died April 24, 2009. According to the civil suit he died from gangrene/osteomyelitis at the wound site and dementia. Martha Shafer, the wife of Ralph, is seeking more than $50,000 in damages.
