August 9, 2010 in Delray Beach, Florida Haile Brockington was left in a Day Care van for hours. Amanda Inman was the driver of the van and had left her in her car seat because she did not see Haile in there.

According to Florida law, drivers transporting children must keep a log and  two other people besides the driver must sign off on the log after a physical sweep of the vehicle is done. The director of the facility admits to signing off on the log without actually looking in the vehicle.

Haile was two years old when this occurred and was in the sweltering van for hours. The temperatures in the van rose to more than 130 degrees while she was in there. Unfortunately, Haile died as a result of the Day Care’s negligence. The parents do have a wrongful death suit filed against the Day Care company and after this incident they lost some of their funding and had to shut down.

Fentanyl gel is 100 times stronger than morphine. The prescribed dosage is once every three days and it is administered by a patch that is placed on the skin.  A recent lawsuit filed against a nursing home involves this medicine and the improper administration of it.

When the nursing staff went to give a man recovering from bone cancer the medication they placed three of the Fentanyl patches on his neck at once. On April 9th Erik Kuentzel brought his mother to the Sunshine Villa Assisted Living facility. Among some of the fees that he paid, one of them was for something called a WanderGuard, which is supposed to monitor where the patients go to ensure they are safe.

Erik’s mother, Carol Fundingland, was 74 years old and suffered from dementia. He told the facility this along with the fact that she liked to take walks.

Ms. Fundingland was found dead three days later. She had died from hypothermia and exposure.
