Kenneth Chin was living in an assisted facility and was being transported daily by a shuttle to a center for adult day services. When he did not arrive home one day, his caregivers immediately contacted the police.

Upon further investigation, it was found that the driver of the shuttle that day was a substitute, did not know the route well, and did not speak English well at all. It was also discovers that Mr. Chin had been dropped off in the wrong place.

A search was ordered and they found Kenneth less than a mile from where he was supposed to be dropped off. The autopsy showed that he died of hypothermia.
